Dear Parent/Carer,
Please note that your child(ren) will be bringing home a nasal flu immunisation consent form today.
Please can you complete the form and return by Wednesday 2nd Oct to ensure that your child is booked in for the immunisation.
ALL forms need to be returned for every child - even if you DO NOT want your child to have the Nasal Flu Immunisation. Should you wish your child to have the Flu Vaccine Injection Immunisation instead, please email the office your child's name and class -
and we will send home a consent form for you to complete the injection consent form.
